I start by identifying the particular needs of the student: do they have problems with understanding some or all of the subject; are there any areas of weakness that need to be strengthened; are there issues with exam technique that need to be addressed?

Even well taught students who have a good grasp of the subject can fail to achieve their full potential through poor exam technique. My earlier experience as a Chief Examiner, responsible for setting and marking A level exams, means that I have a detailed knowledge of the exam system and how students need to approach exam papers. At the beginning I like to get a feel for what level of knowledge students have, and together we set goals for what we wish to achieve (e.g. time remaining until the exam, what grade they wish to achieve, if there is any particular section they are struggling with, etc..). Preferably, I like to communicate with my students before the lesson, so we can decide what learning outcomes to focus on during our session, so we can both prepare beforehand.
